After his brother died Sans became furious and swore himself to kill them, no matter the cost. When Chara arrived at the judgement hall, Sans managed to kill them but Chara's death was rather violent because Sans tortured them to death, forcing Chara to reset and never mess with his timeline again.

Fellswap on the other end takes underfell and its designs, and swaps the caracters like underswap. It retains a lot of features visually from underfell, such as the presence of red. Basically it's the order in which the two aus are combine that changes things.
